Ingredients:
2 cups (12-oz. pkg.) NESTLÉ® TOLL HOUSE® Semi-Sweet Chocolate Morsels
1 can (14 oz.) NESTLÉ® CARNATION® Sweetened Condensed Milk
1 cup chopped walnuts
1 teaspoon vanilla extract
Directions:
LINE 8- or 9-inch square baking pan with foil.
COMBINE morsels and sweetened condensed milk in medium, heavy-duty saucepan. Warm over lowest possible heat, stirring until smooth. Remove from heat; stir in nuts and vanilla extract.
SPREAD evenly into prepared baking pan. Refrigerate for 2 hours or until firm. Lift from pan; remove foil. Cut into pieces.

Here is the most simple fudge recipe I've come across over the years, keeping the most traditional fudge flavor. It has a few more ingredients than what you mentioned, but I think you'll enjoy it!
1 1/2 cups granulated sugar
1 can evaporated milk (5 ounces)
2 tablespoons butter
1/4 teaspoon salt
1 1/2 cups mini marshmallows
1 cup semisweet chocolate chips (6 ounces)
1 1/2 teaspoons vanilla
Grease your desired shape and size baking pan ahead of time.
1. Combine sugar, milk, butter and salt in medium heat, stirring constantly with wooden spoon. Boil 5 minutes, stirring constantly.
2. Remove saucepan from heat. Stir in marshmallows with wooden spoon until melted and mixture well blended.
3. Add chocolate chips and vanilla. Stir until mixture is smooth. Stir mixture 6 minutes or until slightly thickened.
Immediately spread into prepared pan with lightly greased rubber spatula. Cover with plastic wrap and refrigerate 2 hours or until firm.
Now, I usually use this recipe and then modify it to also make a batch of peanut butter fudge and white fudge. I then spread each batch on top of each other in a 13x9 pan to make triple layer fudge. It turns out great!
For peanut butter fudge:
Reduce granulated sugar to 3/4 cup; add 3/4 cup packed light brown sugar. Omit butter and chocolate. Prepare as directed in steps 1 and 2. Stir 1/2 cup creamy peanut butter into mixture as directed in step 3.
For white fudge:
Substitute white chocolate chips for semisweet chocolate chips. Prepare as directed in above steps.

Fantasy Fudge
3 cups of sugar
3/4 cup margarine or butter (1 1/2 sticks)
2/3 cup evaporated milk
1 pkg. (12 oz.) semi-sweet real chocolate chips (or milk
chocolate chips)
1 jar (7 oz.) Kraft Marshmallow Creme
1 cup chopped nuts (optional)
1 teaspon vanilla
Stir together margarine, sugar and milk in heavy 2 1/2 to 3-quart
saucepan; bring to full boil, stirring constantly.
Boil 5 minutes over medium heat, or until candy thermometer
reaches 234 F, stirring constantly to prevent scorching.
Remove from heat.
Gradually stir in chocolate until melted. Add remaining
ingredients; mix until well blended.
Pour into greased 9x9-inch or 13x9-inch pan. Cool at room
temperature; cut into squares. Makes about 3 pounds.

check the label on a jar of marshmallow fluff the recipe is there i usually omit the choco chips and use peanut butter
