They begin with a mold of a Caramilk bar which they place upside down, they pour some chocolate into it, enough to create a layer but still have a little pocket. The chocolate cools, then they pour caramel into the mold, they cool the caramel, and then pour the rest of the chocolate on top. Quite simple and the only real trick to it is that the bars are made upside down.

they make it with the curved side down, let it set fill up with caramel and put the flat bottom on, let set, turn ver adn wrap. But on a huge scale

Ahhhhh....I love those.
First.....the Caramel Fairy digs out the center of a solid bar of candy......then she craps her sweet,gooey caramel into the hole.
Straight from her @ss to your lovely confection!
Then they freeze it,wrap it,and ship on out.
