All at once? Call 911, make sure baby and 5 yr old are safe - stay with injured child. Babies don't die from crying and it isnt the first or last time the 5yr old will misbehave. The medical emergency need your full attention. Call the parents when the situation is calm and dealt with.

Assess the emergency - If you can manage to leave the patient, get the baby - address the 5 yo - plant the 5 yo and deal with the behavior later.

You don't give a particular medical emergancy(is it you or someone else??)...so here's a ball park.
1) Determine your emergancy...if you feel it is out of your hands call 911
2) it's okay to let babies cry...especially in medical emergancies
...annoying but ok
3) ask your 5 yr old for help....i.e. mommy needs your help, can you check on the baby??? Or Can you go get daddy(or neighbor) because mommy needs help????
Give the child something to do to help...as you don't state your emergancy this is BallPark.

1.) 911 and stay calm
2.) patience....babies can sense the stress
3.) time outs if needed....otherwise get down to their level and explain to them that what they are doing hurts you and/or others and they should find something else to do.
I was taught always to use positive discipline....
