1) Go try on rings at a couple ring shops for fun - to find out the right size.
2) The "best" way depends on the girl. She may like "show", she may like "private". Regardless, have a thoughtful design, and if in doubt err on the side of treating the occassion with respect and dignity.

I don't know if this works for all women, but the ring finger on my right hand is almost the exact same size as the ring finger on my left hand (the one you're interested in). If you are planning a bit further ahead and you have a bit of money, buy her a ring for her right hand. Take her with you, make it a cute, informal, "love gift" event. Then you can either make note of the ring size or ask the jeweler on the side what size that ring was. Even if the size isn't a perfect match, it should be an easy sizing.
Getting family / friends to help swipe an appropriate ring or needle the size out of her is an easier plan. We ladies can be pretty sneaky with our girlfriends, especially if we like her guy!

I was told once that for some women, their shoe size is a good indicator of their ring size - I'm a size 8 in shoes and rings, but this could be a coincidence. But at least you have a starting point.
As far as proposing, I always thought anything out of the blew is great. Propose while going for a walk or slip the ring on her fingers while she's asleep (that is if you share a bed on occassion), or place the ring box in a place where she's likely to find it while getting dressed in the morning (medicine cabinet, make up bag, etc).

Invite other women over about the same size and build. (say a sister or friend) and have them sit near her and look at their hands compared to the GFs. Then ask the other woman what her ring size is and maybe have her help you pick it out. Some stores ( ask the jeweler when you buy it) will pay the price to make the ring 1 size larger and if you found a woman with similar hands then she is probably the same size or a tiny bit larger/smaller.

I wouldn't bank on the shoe/ring correlation, but that's because I have a size 7 ring and a size 13 shoe... =/
If she wears other rings, see if you can get one out of her jewelry box (make sure it's one that fits her!) and bring it to a jeweler to get sized.
You could also ask her mom or her friends, if they can be subtle about finding out. If she wears rings, maybe one of her friends can go, "Oh, that's gorgeous! Can I try it on?" If it fits the friend, then you can ask the friend for her size. If it doesn't, the friend can ask "What size do you wear?" in the course of conversation. But the friend has to be suave enough to make your GF think it's about the ring she's trying on, not that she's getting information for you.

I asked my girlfriend her ring size. Then at Thanksgiving Dinner my folks took her and I out for dinner and I proposed on my knee in the middle of the restaurant.
Edit:
Lysa has a great idea. Get the folks in on it. They'll be family eventually, so it makes them feel as if they're involved!

Ask her if you could try one of her rings on, to see how far down on your finger it goes. Then go to a jeweler and tell them what you did. They can give you ring sizers to try. Now when my husband decided to go ring shopping he took me along. I wanted to give him ideas as to what kind of ring I liked. He also then knew my ring size. We went to several different places and he then had a couple of ideas. I still didn't know when he was going to ask me. So you could still suprise her that way. You could also ask her what her ring size is. Just let on as if you're going to buy a ring (birthstone, ect.) for her birthday or Christmas.

One day, my friend decided to take a road trip w/ his girlfriend. On the way to palm springs...he pretended he was lost and asked her to get a map from the glove box....when she looked, her ring was inside the glove box. He then asked her to marry him. She said yes, ofcourse!!! Once they got to the hotel...their friends and family were all there to celebrate their engagement.
Oh for the ring size...ask her mother, sister (if any), or girlfriends...they usually know...or if she has some rings lying around, take it to a jeweler who will measure it for you.

If she already wears rings find the one she wears on her ring finger and make an inprint on a bar of soap...it's what my fiance did for my promise ring. If all else you can get the ring and have it resized later.
