For anything at all to be, or to exist, didn't it first have to be an idea or plan or effort of something ? How could that not be so ?

Why must existence itself have a cause? Causality is a concept we observed with the confines of existence, yet the substance of which everything is composed (energy), is never observed to be created nor destroyed.

Why would it have to be a plan? Do teenage girls actually PLAN to get pregnant?

No, you don't plan that. It just happens.
