How did this all start? With colonialism or, even further back, with the Islamic world missing out on a Reformation, the Renaissance and the Industrial Revolution.
Why do radical Muslims hate Americans? Mostly because of our foreign policy, mainly our often uncritical support of Israel.
What is the real issue? It's complex - a mixture of anger at their being oppressed, both by the West and by most of their own "leaders", some envy, a lot of fanatical mind-sets, and their taking religion far too seriously, much as the West did 400 years ago and before. The year on the Islamic calendar is 1427, and, in terms of European history, that's just about where many Islamic countries are nowadays. They were stranded in the backwaters of historical progress for centuries.
-Why must innocent people die in order to get a message across? Because fanatics, having "a direct line to God", believe that their ends justify any means.
-When will this end? Not in our lifetime, probably. To some degree, it will never end since there will always be whackos who "know" that their cause is worth killing for and dying for.

In Arab nations today, 10,000 Bin Ladens were born. Our only hope is our young people. Look at History, it was our young people who ended the Viet Nam war, they are not stupid, nor are they un informed. They also vote. Our young people can make a difference in how these 10,000 new Arabs see the USA as a people. Will the 10,000 new Arabs grow up with hatred or will they grow up with hope that Americans are good decent people is the question.
Do we as a self proclaimed people that are Godly, good, decent people invade countries after they have surrender? Do we kill 300,000 to 500,000 civilians in Iraq since desert storm? Do we send Israel $2 billion a month to buy arms? Do we really treat women as equals in America? Do we share our vast wealth with the poor in Africa? Actions speak far more than words.

Radical muslims hate anybody who does not comply with their ideology and their lifestyle. I used to think that they hate America only because they have created Israel in Mideast between Arabs; but thats not the only case. Look in Kashmir, why those radical muslim terrorists have killed around 40,000 Hindus till now since last 30 years of insurgency. Remember that state was historically ruled bya Hindu King. The only reason they want it separate because they want to expand everywhere they can by force. They have a pan-Islamic agenda to convert all the regions lying between Turkey and Indonesia as Islamic green. They are working hard to make all the ex-soviet block muslim countries [which happen to be moderates] as radicals. Saudi Arabia is pumping lot of monet there to make themfollow Wahabi style violent and hateful Islam.
So you get the idea of real issue.
The only way you can stop it is fighting them and killing them in large numbers whereever they exist or keep them contained in their own countries.
But if muslims make your life fearful and difficult, do what is necessary to prevent any terrorism in your soil like do profiling , spying on all muslims, etc. As it is proven that a large number of the educated and rich muslims living in western countries are supporters of Jehadists and violent Islam.
How did it all start---it never started from some date. Islam religion itself was born and grew out of fear of sword. See the world history. You know Gengiz Khan, how he slaughtered all non-muslims and anybody uncompliant to him, when his army invaded many countries.
Invading persians and Arabs converted millions non-muslims to Islam by violence and force in South Asia specially India.
The Asians knew since centuries how peaceful Islam religion is? The western world did not know or they ignored it. But now when they started touching their lives, they are getting the taste of peaceful Islam too.

One question is why the NON-radical muslims are loudly vocal in condemning the violence. Timid decency=approval and support.
