They might not notice any changes to their health, however if they did notice things you might find them to be something like the following:
Physical - Typically the material injected is not quality controlled. Conditions are not sterile. Unsurprisingly these conditions result:
Infections in the arms at injection site
Inflammation of the veins
Heart disease - endocarditis - with infection of the heart valves
Septic emboli to the lungs from showers of bacteria from infected heart valves
Pneumonia - cavitating abscess formation from the septic emboli
Mental - the nature of the drug is addictive. The come-down is what gets them motivated to have some more because the high was so good.
Physical and behavioural addiction - they get into the habit after a while. The body becomes very good at metabolizing this crap they inject and so you need to have a bigger hit, better hit, more frequent hit. Then the habit forms after months. They feel anxious without a hit and will hit up on ANYTHING - will inject water (bad because of osmosis and the red blood cell) or anything they could think of.
Behavioural situations can worsen with impairment of judgement, depressed mood, desperation to get another hit, the commitment of crimes eventually to pay for the heroin, arguments and family difficulties, relationship breakdown, trouble with police and with drug dealers ...

If shooting, It's called chipping... it is the last rational resort of a person about to become a full blown addict without ever seeing clearly enough to admit it to themselves or believe in the possibility of it coming to that extreme.
If snorting... It is the start of chipping.
Heroin is like a smooth lover... it will romance you slow and precise, subtly, ritualistically and on a constant, right out of your own common sense and eventually, your life.
It is how every heroin addict starts out. It is like Kindergarten for the whole junkie education process...
The only rational thing for them to do would be... STOP WHILE THEY CAN! Health effected or not! It happens eventually.
Don't play with your head like that... no matter how strong you believe your resolve is. The White Horse is always stronger.
*Don't listen to that little twit below me either. She has obviously never used, and to say something like that to another human being if she ever has used... is morally deplorable! She sounds more like a dealer than a user to me... either that or another person slowly becomming an addict but still completely convinced that she can handle it...
a wife and mom too... that's nice!

What happens? You start at once a week then it is 7 times a week. After that you start selling stuff to get money. There is no such thing as "casual" heroin use. Best bet is to never use it again but it is probably too late for that advice.

It may be once per week but very soon it will be multiple times per day.
Addiction is like that.
They will loose weight and loose the ability to care for themselves.
They stop bathing and eating and almost everything except using heroin.
I have a Friend who had been an addict for about 10 years.
He has hardly any veins left that he can shoot into. He has had pneumonia several times and has heart problems and skin infections and other problems.

there is such a thing as a casual heroin user, just the same as a casual crack user. I WAS both, i recognized when the addiction would start and i stopped for awhile to let the tolerance go away and then would continue. if you can handle your use and control it instead of letting it control you...have fun
