This is what I used when i was doing daycare
Top of the sheet name and date
Three diferent areas
one for diaper changes/ potty breaks on wich you write down the time and if they did BM or Wet or nothing
Another area for meals waht time they ate what they ate and how much, dont need to meassure just write dowwn A lot, a little didnt eat tried everything, stuff like that
Another area for sleeping patterns, how much, at time thay went to sleep and what time they got up At the bottom of the page i had an area for other comments where you can write down whatever , if they were happy or excited or if they fell or did something new.
i know you might not have a lot of time so my sheets were mostly check maks and i kept them above the diaper changing are.

I make a newsletter for my staff using Publisher a part of Microsoft office. It is very easy, you just follow the prompts. Click on to publications for print. Scroll down to newsletters choose which design you want. Then edit the amount of pages you want eg 1-4. The columns are preset areas for headings etc. Give it a try. Just work through each section it is really easy.

i would go into word pad , and make it your own style
played well with others- 3 boxes-good-fair or sad face
ate good--happy face-straight face-sad face
you get the idea, good luck
