A perforated eardrum, particularly if it's new, certainly can cause most of the earlier symptoms you described. You said you did not have any other illnesses, but have you checked your blood sugar? You may have some form of diabetes.

How old are you? Do you drink coffee or energy drinks and/or take any herbal supplements? Is there a time of day that you seem to experience more of these symptoms or is it fairly consistent throughout the day? Have you noticed an decrease/increase in appetite? Weight gain/loss? Insomnia/fatigue? Changes in skin/hair/nails? You could have a thyroid condition, but I need more information to rule out other possibilities.

Sounds simillar to carbon monoxide poisoning,,or could be something in your apartment,or place of employment,or vehicle.
Carbon monoxide is not the only gas which can cause these effects.
Another cause could be mucous build up due to cold or allergy,,this will make free breathing difficult and starve the body of oxygen.
Get a second opinion from another doctor to be safe.
