An example of dead wrong would be this: a car approaches the railroad tracks just as the lights and bells go off. The driver doesn't want to wait for the train to go past and he is sure that he can get around the barriers quickly before the train comes so he tries to drive across the tracks but he has miscalculated the speed of the train. The train hit his car, killing him. Not only was he wrong about ignoring the warning not to cross - he was dead wrong!

It has nothing to do with being physically "dead." It's the same idea as when you're saying you're "dead on." The "dead" means "straight" or "completely" in this case.

dead can also mean exactly or definitely because death is something that is definite in a tangible sense, therefore if you're dead wrong then you're DEFINITELY WRONG!
