This is how my Italian Uncle taught me how to do it. He owns a bar and I bartend for him and the club goes WILD!
Get a glass - do not put ice in it yet!
couple of dashes of worchetschire (sp?) sauce in the bottom
salt & pepper
celery salt
1 shot and 1/2 of vodka
Then add the ice to the top (for some reason, doing all this first makes it taste way better than adding ice and then adding sauces.)
then fill the rest of the glass with V8.
If you want it hot, put a couple dashes of tabasco sauce in it.
DO NOT GET THE PREMIX! IT SUCKS!
Then repeat the first 3 steps on the top. WA-LA!
you could garnish it with a couple of olives, pickles or celery. makes the crowd go wild!

Dear Andrea,
Use a combination of 2 cups of Clamato or Tomato Juice, 2-3 drops of worcestershire sauce, 2-3 drops of tabasco, salt, fresh ground pepper, a capful of lemon juice, and enough chunky horseradish so that you can see some chunks of it floating around (or less, if you don't like it so hot). Rim the glasses with celery salt or Old Bay seasoning. Add vodka to taste (not too much so as to overwhelm the tomato mix) and shake.
