They lost one near the harbour of Savannah GA when a fighter hit a B52. The military never recovered it but a civilian team located it. It's still there
They lost one in the Atlantic near Spain and one in the Med. A B52 went down some where in Alaska.Those are documented and there are some rumoured. Frankly if you add them all up it seems that your military is as careless with nuclear devices as the Russians but at least when they lose a nuke someone makes a few rubles. What is really scary is the attitude. The one of that is off Savannah,although very dangerous, has simply been written off and "no more funds are available for retrieval of the device".

Yes! It was dropped off of the coast near Savannah. In 1958 a plane doing practice drills in Savannah had some problems with their landing gear. The plane had a nuclear bomb on board. It was decided that it was better to drop the unarmed bomb off of the coast than to risk having it spread out in a crash. Although many people have looked for it, no one has found it and they haven't had any signs of radiation leaks.

The US has lost several nuclear weapons due to aircraft crashes, submarine accidents etc.
As you might imagine, the locations are not widely advertised.

Not that has been reported. Even if we did lose one, I very much doubt that it would be reported by the government. Too much international and domestic chaos.
