My mom thinks everything pretty much has to be refrigerated once opened. I hate cold ketchup so mine sits in my cabinet :)
At diners they keep it sitting out on the tables. No place that I've seen on the bottle says to refrigerate after opening. So....
I'd say you're safe both ways. :)

Yes, I do believe that it does. Check the bottle. It should say somewhere on the label.
Ketchup, mustard, relish, jams, all need to be refrigerated after opening.
Fast food joints put the condiment packets back in the fridges at night. They can stay out during the day because they won't go bad as fast as mayo and dairy products would. Plus, they probably have more preservatives in them.

Yes it should be to keep it tasting fresh but if you go to restaraunts they have it just sitting on the table so either way goes i guess. I like my ketchup cold but some people like it room temperature

No, but it stays fresher longer if you do, and it tastes WAY better. It will start to get thick if you leave it our too long.
