where can I get a copy of admission forms to a hospital?

at the inpatient records/outpatient records at the hospital, they need written consent so you'd need to go there

Ring the hospital that you are interested in, ask for admissions and they will send you the forms in the mail, Take care.

call the hospital and ask for medical records and tell them u need a copy of the admission forms for, what ever date it is you need them for
