As far as history is concerned all presidents have. Only one though did not use the word 'swear' and that was Franklin Pierce who had one hand on the bible and the other on a book of law and he used the word 'affirmed'. Most historical records say Pierce did this because he was in a crisis of faith because his last remaining son was killed in a train accident a few weeks before the presidential inauguaration.

But, there is a difference.
They put their hand on the Bible and swear to uphold the Consitituion. They do NOT put their hand on the Constitution and swear to uphold the Bible.
BIG difference folks!

They all did, probably. But, just because all the presidents had to do the traditional thing, doesn't mean they had to believe in it. I happen to know for a fact that Taft was a pastafarian.

Even the accounts of George Washington doing so aren't supported well. The reporter who supposedly "saw" him do it wasn't there at the time.
