OK, I am invited to a co-workers college graduation. He actually graduated about a month ago, but they are just now doing the ceremony. I already sent him a card last month after he got his diploma. Do I buy another gift or card now too?

How "close" are you to this coworker? If you're not too close, I would say you fulfilled your "obligation" by giving him a card earlier. If you are close, then, yes, I think a gift would be in order. It doesn't have to be a "big" gift- maybe a gift card to a favorite store. There are cards now that one can place a gift card inside.
