I can't really speak for worldwide, so I'll confine my answer to the Church in America.
1. Poor catechesis (ignorance/misconceptions of dogma)
2. Infiltration of society (especially an individualistic/relativistic mindset)
3. Spiritual torpor (lack of interest or fervor for the Faith)
(4. Deliberate dissent, self-serving, and grandstanding by both the laity and clergy)
Pretty much in that order.

1. The idea that the Bible is 'factual'.
2. The idea that God is somehow separate and external from ourselves.
3. They are in bed with Republicans and they are destroying their religion.
