Usually a jolt on the train and a loud ratcheting sound when the wheel begins to hit the cross ties. A train seldom derails as the wheels are so well balanced that the flange on the inner part of the wheel is merely there for "emergency friction" and to help it get through switches. The most common time for derailments are when the weather is extremely hot and the rails get so heated that they expand and begin to bow.
