place some two-sided tape (like packing tape) on the edges or wherever you think he will land. cats hate for things to be stuck to their paws. or maybe tie a ribbon around his waist. most cats lose their balance when you do this. that way he knows that if he is going to jump on your counters, you are going to torture him like this...lol
let me know if it works for you

Hi there...one person here already has made the best suggestion, which is to use double-sided tape. By squirting or chasing the kitty away it teaches him that you are the aversive rather than the area you don't want him to be in. If you are no where present when the kitty is caught in the tape, he will associate the bad effect to the area rather to you. However, in order this to be effective you will need to leave and replace as needed the tape for a couple months. Cats are naturally inquisitive and will return to investigate if it was just a single incident. Additionally, if this area is kitchen counters it's important to regularly soak or clean all the dishes so this will discontinue tempting the kitty as they can smell the left over food.
I've included a website that explains in detail how the squirt bottle doesn't work well and why.

You can get a long piece of cardboard about the legnth of your counter and just balance it on the edge of the counter, then take two empty cans and fill them with some coins, or marbles and place them on the cardboard. When your cat trys to jump on the counter he should hit the cardboard, which will then fall to the floor along with the cans of coins/marbles, which will scare the cat...it will only take a few times of this then your cat won't jump up on the counter anymore
I did this with my cat on the dinning room table and I only had to set it up like 3 times and he hasn't been up there since.

Use a spray bottle that shoots at long distances. Use cold water and spray the cat when you catch him on the counter. It works wonders. You may have to do it for a while but as long as it can shot water pretty far your good to go. It doesn't hurt them, but everyone knows cats hate water.......

If the water spray is not deterring him fill a soda or coffee can with loose change and shake that when he jumps up.
Cats hate loud noises and are truly frightened of them. Say a loud "NO!" too.
Then with the cat always use the NO and no other verbal command.

I clap my hands loudly and say No! loud and firm as soon as my cat does it. She doesn't do it anymore. The key is responding as soon as they jump up there. If you wait, they won't understand what you're upset about.

Keep spraying him, telling him no, and start talking to your vet to get some ideas to train him.
