They claim it was a medical appointment, yet I have no record of making a payment at the office and my insurance company has no claim form submitted for that date of service. The date is over a year ago and I never received a bill.

Per the Fair Debt Collection Practices Act, you should send a letter via certified mail, return receipt to the collection agency disputing the debt. The collection agency has 30 days to respond to your letter with proof of the debt. If they do not, they cannot further collect on the debt. Keep a copy of the letter and the receipt for your records. If you continue to have problems with them, contact your state attorney general's office and the federal trade commission to file a complaint.

this sounds strange. You need to get in touch with the medical billing office that the doctor has. The should have all your information in there computers to let you know what is going on. If you so owe monies....if you send 5 dollars a month they will not be able to do nothing to you....best of luck to you....ps i do billing for drs....

You can request in writing...sent via signature receipt requested, a letter asking for proof etc. If it's on your credit report you would file a dispute with the credit bureau.

Dispute it in writing and send it certified mail, return reciept requested. Ask the collection agency whom to send legal documents to and the physical address to them to. State in your letter that the debt is not yours, and you are in dispute of the balance in full. It is up to them to prove it is your debt within 30 days.

If it's on your credit report you can disput it through experian.com. Put everything is writing and mail it to the bill collector and the MD office.
