Eboli, blood pouring out of you would be pretty bad. Or the flesh eating disease. Heck, just knowing your gonna die is pretty bad.

I know this because I seen it in science class, it's definitely Ebola. It's the also called the black plague. It's absolutely disgusting, I saw real cases of it found in South Africa on this video my teacher made us watch. People were put in quarantine because it was highly contagious. They literally bled to death from every opening of their body. It comes from a bacteria carried by Monkey's and rodents, not likely found in the US unless it is carried here. On the video people weren't even given a funeral because it could still spread. So drs dressed in plastic space suits dug them graves, wrapped them in plastic, soaked them in bleach and buried them deep. Pretty bad huh!?

Definitely ALS (Lou Gehrig's disease). You basically die from drowning in your own fluids.
Huntington's would be bad as well. It basically causes you to go crazy, slowly.

Parkinsons's would be pretty bad because your mind still functions, but your body fails.
Other than that, I would think a serious cancer where the last year of your life totally sucks and you're in constant pain.
