My doctor's office closed a few years ago. I now need my medical records from them. How do I find them?

You can contact the AMA and give them the name and address of the doctors office at the time of your service. The doctor should have contacted you before they closed and given you a choice of having them transfered or being mailed to you. Those records legally belong to you.

If your doctor retired and someone accepted his practice you were notified and had a time frame for requesting your records be sent somewhere else. Generally speaking records are available for transfer or pickup for a specified period of time. When that time has lapsed those records are usually destroyed since no one claimed them. Keep in mind that when you ask for records you get a very slim version of what was actually part of your original file. Only the highlights are part of that transfer. Meantime what kind of a screwball would think the hospital where you were born would have your adult medical history on file. I'm afraid you are more than likely going to have to rely on memory for the most important events, better start writing.

More then likely the practice was taken over by another doctor. Do you know what hospital the doctor was affiliated with? You can call medical records there. If you still have the old phone number you can try calling it, it could be forwarded to another doctor's office

Do you know where your Doctor went? If you can find him then you can get your records. State Law they have to keep a copy of your records with them for so many years, I think at least 10 . When you locate him and find a new doctor give the the info. and you will have to sign a release form in order for 1st doctor to send 2nd doctor records. Good Luck. Hope this helps.

Check to see if that doctor was replaced by another doctor your records might still be in that office. If not, maybe your local hospital might have those records. You have all rights to get your medical records. Good luck on your search

They probally have a main office someplace still. You will need to research it and find out. Or did another doctors office take over? If that is the case then probally that drs office has you in their archives. Especially if you have not been there in a few years! You will then have to sign a form or write a letter to get permission to get the records.

Were they ever transfered to your new doctor. Call the local division of ama or physican association and ask them how you got about getting them from a closed office. They ahve to have them some where for you to get at because by law they have to keep them a certain lenght of time.

Surely he/she had a business partner, or perhaps sold the practice to another Dr. Call your local hospital records department and see if your Dr had privileges there. You may have to pay a small fee to obtain your records.

well you can find the doctor that was in charge if he is still their or call a hospital and find out were the records may have gone so y ou can obtain them
