I am answering this question as a native-born son of a family which fled France in 1657 to the United States.
I am answering this question as a Muslim who has been to Mecca and has completed his Hajj.
First, dropping a nuclear bomb will probably kill millions of innocent men, women and children. Millions more if dropped at the time of Hajj. This sort of genocidal mayhem is terrorism, just like the dropping of the bombs on Hiroshima and Nagasaki.
If wholesale slaughter is what you are after then it would be effective.
If you are wanting to destroy Islam then you will do the opposite.
The Ka'aba has existed for since the time of Abraham; yet it has been destroyed by war a few times. This is why only Muslims are allowed within the area surrounding Mecca, to prevent this from happening again.
What will happen, *IF* the Saudi government doesn't shoot down the plane carrying the bomb (the area over Mecca is a controlled flight zone) and the bomb does indeed blow up.
All the 73+ sects of Islam will lay down thier differences and become unified, there will be a leader appointed - the re-establishment of the Caliphad - and a physical aspect of Jihad will be declared.
Instead of a few radical groups what you will have is 1.3 Billion Muslims practicing the ordained defense of thier faith. You will have 1.3 Billion Muslims worldwide ready to decimate whatever army dared to come against Islam.
We Muslims are indeed a peaceful people. There are less than ten thousand persons involved in radical terrorist-like groups around the world out of 1.3 Billion people.
Do you really want 1.3 Billion people to have a reason to become a unified army?
Though, after the war of this magnitude it is likely that the world would be enveloped with peace. Like Saladin enforced in Jerusalem after reclaiming it from the Christian armies during the Crusades; he threw open the city gates and sent messengers throughout the land to let all people know that they were free to come back and worship in the holy city.
I am an American and I am a patriot. But if my country were to do something as an assault against my religion I would not tolorate it. My God and my Faith are always going to come before the politics of my nation.

It's an old question, but I think if the US nukes Mecca and Medina, they have to wipe out every Muslim country otherwise their will be uprise of over 1 billion Muslims worldwide (as pointed out by Hajj Saifullah which got the best answer). And even that is not impossible to do. The United States has more than enough nuclear weapons to destroy the entire world several times, not just only Muslim countries. So, if they decided to do this, then it will be a complete (religious) world war. Christianity vs Islam. In the long run, technologically speaking, even with the billion Muslims on Earth, there are also 1.5 billion Christians, the West would win this war. But that is only due to it's technological advancements over Muslim countries. Let's hope this situation will never happen.

The same thought has crossed my mind, barbarism as a response to barbarism is a futile exercise however the bombing of Japan ultimately led a backwards feudal society into the 20th century and maybe the same could be said for Islam. I think the choice of Targets could be maybe Karachi or Syria to calm these religious zealots down and maybe this would lead these head choppers to enlightenment much like Japan.

Read the history of Islam from 700AD to about 1700AD. You will see none of this is new....ISIS is not new. Islam has never been, even for one day, a religion of peace. This is the effect of the Stone.
The history of the Black Stone is largely hidden. It has been around forever. It is pre-Noah. You have to dig for the its history, but you will see wherever it is, death, war, mindless destruction and genocide are also.
God destroyed the world for a reason, whether you believe it or not. The stone is a remnant of those times of unmitigated evil. Its affects are devastating.

IF the US nuked those two places, I would laugh and be very happy. We'd take away their two most holy places, and one of the requirements (visit mecca) of islam.
I think every muslim in the world, though, would rise up and work for the death of every american and, probably, non-muslim in the world. they'd be pissed. but that'd give us a good excuse to nuke the whole middle east...get rid of the plague.

Well I am thinking that you might as well nuke most of the world, because there are Muslims in almost every country, and the backlash from such an action would make today's threat from terrorists look like a cakewalk by comparison.
