Your brother's male child is your nephew. Your nephew's children are your great-nieces or great-nephews (in genealogy I don't believe there is a "grand-nephew"... "grand" is usually reserved for grandparents).
Your brother's children and your children are 1st cousins. Your brother's grandchildren and your grandchildren would be 2nd cousins. Your children and your brother's grandchildren would be 1st cousins once removed (different generation level).
Clear as mud?

Grand-nephew or great-nephew. I call my mother's aunts my great-aunts because that's how my mother did it but now that I have children I teach them that my aunts and uncles are their grand-aunts and uncles because I feel they are equal to the grandparents.
Your grand-nephew would be equal to your grandchildren.
