"feedback" is responses to an action or statement, which are collected and used to determine if any changes need to be made. A simple example is: any answers you get here are feedback to your question. Another example: a boss can't decide whether or not to extend the office lunch hour. He sends out a memo to all staff for their opinions on the matter, requesting a response. The responses are then considered as a whole, and he makes his decision based on the "feedback" from his staff.

That depends on just what you mean, feed back happens when your to close to your amp and a resonant "loop" happens and you get distortion of the sound out put. Or quite possibly an answer to a question you posed like what I'm doing right now, not all feed back is positive.
