I think I'd have to go for The South Bank Show theme. A Rachmaninov variation of a Nicolò Paganini tune arranged and produced by Andrew Lloyd Webber and performed by virtuoso musicians from Jazz, Rock and Classical disciplines. It's brilliant.
The worst is the thing they used for 'Enterprise'.

C'mon! Everyone knows it's the theme from Gilligan's Island. I don't think there is a person over the age of 9 that doesn't know every single word and every single character.

Two of my favourites are "Moonriver" by Henri Mancini for Breakfast at Tiffany's and "The X-Files" theme by Mark Snow.
