I am not a doctor, the only way I can explain it, they hurt because they're getting relaxed and the blood flows different-lying down.
Then he sleeps and doesn't feel anything, but as soon as he wakes up they hurt again. I think he has circulatory problems that should be investigates ASAP. There could be clots, varicose veins etc.

its probablly the bed or the way he sleeps. that happen to my neck cuz my bed and the way i sleep on it. you might need that shapes your body while you sleep. or that other one you always see on tv. give him foot massage in the day and night to help if you cant afford those beds. and get him those water heater things that bubble like a mini foot jacuzzi. And get him that little mashine thats like a weird shape almost like a trunk of a tree with 3 balls that vibrate and roll around to massage but for the feet.
