Just make it up as best you can. They can't fault you for forgetting. If you are off by a month or two they will forgive you and if they don't then it is their loss.

I do not know how much time you have for this but here is my suggestion: Request a copy of your transcripts for each year from the IRS.

You can call 1-800-829-1040, I believe. Keep pressing zero after the prompts and you'll eventually be connected with a "live" person. You can tell them what you need and they will connect you with the right department. The only problem is that you have to wait for the transcripts to arrive in the mail.

If you received a W-2 or a 1099 in the past, the IRS will have a copy. This could be a great way to list all of the organizations or companies that you either performed work for or were employed by.

I am not sure how many years you can go back to however.

A government clearance will go back until you are 18 only(so that saves you 2 years of having to remember things :>) It also generally only goes back 10 years if you are over 28. Call your previous employers and ask for your dates of employment. You can also get some of your old tax returns if you can not remember some of your recent employers.

You can contact the former employer but they don't have to tell you. It is your responsibility to keep up with that sort of thing for future applications.
