Get it with the office of the Foreign Affairs Department nearest where you live and bring the necessary documents such as birth certificate. The fee is very minimal.

You should be able to find a form to fill out on a website www.travel.state.gov. You can download it and fill it out and bring it to your post office. You will also need to get a passport picture taken and Wal-Mart photo center should be able to do that. The cost should be between $60-100 I think.

you will need your drivers license, your birth certificate and your social security card
be prepared to spend about $100
if you have your own picture you will save yourself a few dollars

local post offices can issue passports. it is about 80 dollars and it will be cheeper if you bring your picture
