In forming a non-profit corporation, is it necessary to also have a charitable trust in addition to by-laws & articles of incorporation?

I have just formed a not for profit corporation, NEW FIVE STAR MILLENNIUM ENTERTAIUNMENT, INC. for theater/stage and I was not asked that question by the attorney's. Although, articles of incorporation are usually conducted on a state by state basis, I would suggest you find out first before incorporating.
