I have been trying to get an application for a handicap sticker (parking) for the state of Ohio.

They are available from any license agency where you would get license plates or your driver's license. You will be required to obtain a doctor's signature in Ohio and he/she must state what the disability is and how long it is expected to last.

Not sure about Ohio but in Utah I called the DMV they mailed the form to my mom she filled out her part and had her doctor do the rest then took it in and they gave her the plaque.

My mom had to get a handicap sticker, and we were told to ask our doctor for one. He signed the slip, and mailed it in for us. Call your doctor about it!
