How to make a concrete ramp?

THIS A HANDICAP RAMP. CURB TO SIDEWALK. CURB IS 6" HIGH. NEED TO FIGURE OUT HOW TO KEEP CONCRETE FROM RUNNIG OUT OF THE FORMS.
