try practicing in front of a mirror or with someone else. Try not to feel embarrased. Go up to him, and start a normal conversation with a topic of interest, say soccer! Flip the conversation with,
"what are you doing later on?". go from there, ask him out. The least he can do is reject the offer.

Walk up and talk to him. Ask him if you can call him if you are more comfortable on the phone. Just let him know and don't act like a silly school girl. (Oh yeah, you are a silly school girl)
I can not tell you how many girls acted like this to me years ago, and I never knew they liked me. Talk about a waste of opportunities.

Same as above. Soccer...i don't think so.
Either way, don't be embarrassed. If you think he knows, why don't you gather up the courage to go up to him and ask him if its true. If not, maybe you can be friends or something.
