"President Bush" is the correct title. One may also address the President directly as "Mr. President". It is not incorrect to call the President "Mr. Bush", particularly when the context is already clear (that you're referring to the president and not his brother or father, for example).
In most instances, omitting the title is a sign of disrepect, albeit mild.

It is normal for the media and other sources to refer to the President as, "Mr. [Last Name]," "Mr. President," or "President [Last Name].
The first reference would be used when speaking about him, not to him. The other two can be used in either case.

All Presidents, past and current can be called Mr President, or President_______, but there is no hard and fast rule as to how they should be referred to. Only when you're speaking to them directly, you should call them Mr President or President_______ as a matter of respect.
