When it comes to painful sex in most cases it comes down to not enough sex [foreplay] prior to penetration, or not being emotionally ready, outside of that it is often down to psychological or physical health problems.
That said however with what you are describing it sounds obvious – and frankly I am very shocked that this was only mentioned the once – it sounds as though he is hitting your cervix when he is penetrating you. The cervix is the opening to your uterus, this is just at the top of your vaginal canal and is very sensitive – when you are fertile or menstruating your cervix will be softer, lower and more sensitive, so you may find it more painful to have sex around these times of your cycle.
This has nothing to do with size, it is to do with penetration, vaginas are on average around 3-4 inches relaxed, 5-6 inches when aroused – take into account then that the average penis length is around 5.5 inches long – so if the penetration is too deep the penis can be too big to fit into the vagina, also if not fully aroused this can make the situation worse. During sex in most positions the penis does not go all the way in, however positions such as girl-on-top it will go further in, thus meaning more likely to cause you pain.
In short, try different positions where the penetration is not as deep, or where you can control the depth of penetration a little better in case he gets carried away, of course he should be able to control himself, if he cannot then work on it together. Don't just put up with the pain either, as obvious as that seems the number of girls and women I have come across who seem to believe it is supposed to hurt or that they should suffer pain for their partners pleasure is shocking. The pain can also be pleasurable if you get it controlled in a way that isn't going to leave you unable to walk – I have been in this situation with a past partner, where literally I could hardly walk.
The pains you have after can be eased off in a similar way to menstrual cramps, drinking raspberry leaf tea or a hot water bottle on your stomach, even clarity sage rubbed into your stomach will help as well. The pains you get after exercising are likely to be cramps from lack of oxygen to your body, the pains are similar but not the same thing – although to be on the safe side it may be worth mentioning this to your doctor just in case.

their could be a number of factors that are causing this pain-
1. this size of his penis
2. you could be in the develpmental stage of endometriosis
3. you could have what they call p.i.d.(pelvic inflammatory disease0 dont worry nothing a few antibiotics wont clear up..
which in case can all be serious if you dont go get checked out....a pap smear will rule out anything harmful and at the same time can catch something before i t gets worst..i suffered from painful intercourse for years and painful periods and went and got checked and told me that i had mild dysplasyia..which is small cancer cells forming on my uterus..well long story short they got to it on time and i am now free of the long painful symptoms...we should not have any pain at all..GOOD LUCK

You could have a retroverted uterus. This would cause pain during intercourse. I know you've heard it before but go see a Gynnie and have this checked.
