everyday we all make choices that will effect our lives one way or the other. therefore due to the power of our choices, we are responsible for our actions and decisions. even if we are incarcerated or being held against our own free will, we still make choices that are ours and the consequences of such choices belong to us.

No, every person is ultimately responsible for their own actions. Even if another person held a gun to your head to make you do something you do not want to do, it is still your option only whether you will comply or not. Results from forced compliance will resolve you of any negative outcomes, but it is still only you who makes any decision affecting your behavior or actions. Consequences are only a result of the circumstances surrounding those decisions or actions. A very profound question. Kudos!

As an attorney, I know there are a number of instances where one is exempt from culpability. As mentioned, insanity is probably the biggie. One can be insane or temporarily insane and not be held responsible for their actions. Additionally, if acting under duress, that can be an excuse, though not always. That is, if someone is literally holding a gun to your head and tells you to do something, you can be excused from responsibilty. Those are probably the two biggest examples.

Like any good modern day citizen I am responsible for all of my decisions that had a good outcome and put me in a favorable light, and any decision that resulted in bad publicity, overwhelming negative feedback, arrest and/or grand jury investigation, was not my fault - I was only doing as I was told and I don't recall the specifics. It was a long time ago. I look forward to defending myself and have confidence in the justice system.

I think, that we have to be always responsible for our actions. I am trying hard to get an excuse for the wrong actions I make, but its just like lying to my self.

Yes, you are, unless you can establish that you are legally and functionally insane. Even in this case, you will bear some responsibility for your actions in the sense that you're likely to have your freedom severely curtailed. "Responsibility" does not necessarily imply legal consequences, they could simply be moral or social.
