This is said because, incase of some serious cut which you will be having, maybe at your back or you are unconscious and you are taken to the theatre you will be undress before sending you, and they release you are wearing dirty painties and underwears it will be a disgrace to you.
As a normal human being you have to be neat all the time to avoid an infected diseases.

Because if you get in to a serious accident (car, bus crash, etc.) you will be taken to the hospital. There they will cut off your clothes and will see you have dirty underwear.
Or, heaven forbid, you will end up in the morgue and, again, they will see your dirty underwear.
It's an old modesty thing. Wear clean underwear just in case.

BC our parents always told us that growing up and I know I would hate to be embarrased with some nasty underwear if I were to get caught up.
