You can break the information down into 7 categories...
1) Personal contact information (address, phone, etc.) going back at least 4-5 years
2) Other credit history information that is reported with the major bureaus including credit cards, car purchases, etc.
3) Credit information that is not reported such as rentals, etc.
4) Personal fincances including bank accounts, investments, etc.
5) Any explanations for existing bad credit.
6) Overall current dept obligations (balances, payments, etc)
7) Last but not least, your income (wages, investments, interests, etc)

You will need your basic info, name, address, social security number, and possibly even the amount of money you make in a year. That's pretty much all the info I have ever had to put on an application.
If you are a college student and are applying for a college student card, then you will also need your university name and ID number there.
