This is off 16 Blocks, if anybody has seen that movie then they should know the answer.
You give the car to your best friend and let him take the old lady to the hospital and you stay with the guy/girl of your dreams and have a really great time.

Oh, I went to see 16 Blocks yesterday (the movie, right? ;) ). There Bruce Willis gives the answer:
You get off the car, and your best friend and the elderly lady go away in your car. You stay with the girl/guy of your dreams. Then, (I can't remember the rest very well, but I think it goes like this) someone will come for you and the love of your life, but if they don't, at least you are with her/him.
