Make sure you're using the correct eye. I know that sounds silly, but everyone has a dominant eye and it's not always the same side that you write with. I'm right handed, but I have to shoot left handed because my left eye is dominant. It improved my aim 100% when I switched. With both eyes open, hold one hand straight out in front of you with only your index finger up. Center your finger on a small focal point (a candle, cord from a mini blind, etc.). Close your right eye. If the finger stays centered, you are left eye dominant. If you close your left eye, and the finger stays centered, you're right eye dominant. It may move for both eyes, but the open eye where it stayed closed to the focal point is the dominant eye.

This is the way that we teach it in the Army, and it has always worked for me.
First, breathing. When you breath out, you have a natural pause before you breath back in. Use it. Breath in, breath out, count to three (one-one thousand, two-one thousand, three-one thousand). While you are counting, SLOWLY squeeze the trigger. The round should startle you when it goes off. If you have not fired by the time you hit 3-1000, MAINTAIN pressure on the trigger, breath in, breath out, and start squeezing the trigger, again counting.
Second, sight alignment. The top of the front sight should be flush (even) with the top of the rear sight, and centered in the notch. DO NOT focus on the rear sight. Focus on the front site. If possible, shoot with both eyes open.
Third, trigger pull. Do not PULL the trigger, squeeze it. If you pull the trigger, you will throw the site off. To practice, have someone put a dime on the muzzle, and then practice squeezing the trigger. Keep practicing this until you can "shoot" and the dime doesn't fall off. It's harder than it sounds.
Once you can combine these three things, your shot pattern will come together

a few pointers....but it would help if you specified weapon and caliber.
both eyes open
know your trigger,where it breaks and how hard to pull, get a trigger job if needed
check the muzzle crown...a dammage crown can cause bad groups
use hearing protection and glasses...you will not be affected as much by recoil, or preceaved recoil, alowwing you to concentrate on the target.
RANGE TIME....see what works best for you, and try new things, put trigger finger further on or further off of thigger.....

just rember when you are going to shoot a animal shoot it in the heart as soon as you shoot it it will fall to the grown and if she you shoot it in the lungs it i will run for ever in till maybe the next day and just start shooting with a 22 after you get a little beter start shooting with a scope and bigger guns use the rite eye the right one open and the left close and hould you breath

get a smaller caliber rifle like the Henry hr17 and practice practice practice, then move up to to your larger caliber gun and practice be sure to squeeze the trigger and not pull the trigger.

go practice just go out in the county if you can and shoot at random things. thats how you get good once you know your gun till you know your gun shoot at targets so you can see where the bullets are going in relation to where you are aiming so you can correct for it/adjust the sights.
