It's only one day a year, right? Remember that as I say this: you know that one sexual thing he really would like, but it's kind of messed up, or not to your liking, or whatever? Do it. Do it as if you'd been dreaming about it for years and years. Do it with absolute abandon, understanding that this is once a year, for him.

When you're through (or he's through, more likely), tell him that you love him, and that what just happened is a birthday present, not a new regular adventure. Tell him with a smile and don't use it later in a negative way.

First thing is find out what he wants that always helps. You would be surprised most guys are just happy that some one remembers. It doesn't have to be expensive or flashy as long as it come from the heart. What ever you do he'll appreciate it.

If if was me I would rent a small, little cabin in the woods/mountains, drive him there, pick up groceries/beer/wine,cognac, light the rock fireplace, give him a foot rub, cook him a fabulous bar-b-q, give a full-body message, and then...........................
