Back in the days when I was growing up milk was delivered by the milkman in bottles and was not homogenized and therefore the cream rose to the top. If you wanted whole (homogenized) milk you would have to shake it thoroughly to disperse the cream throughout the entire bottle. If you wanted cream for whipping you would just pour it gently off the top. The remaining milk would then be what we now call skim milk. Nowadays there is no need to shake the milk container before using.

if the milk has been sitting for along time, some of the cream may seperate to the top, so shaking it will mix the cream back in. But usuallly its fine without shaking

You CAN shake milk if you want to.

I think all commercial milk in the US has to be homogenized so I don't think you must shake it. However, don't let it expire, or else not only will it seperate, it'll stink!

Since homogenization you don't have to. But some think the resulting aeration makes it taste better. It really doesn't matter.

only if you like your milk frothy and bubbly.... milk isn't a suspension therefore no shaking required

no people just do that so this it is nice and foamy when you pour it in a glass. SLuuurp :)
