I've been struggling myself to achieve that aim also, but you can't be descipline and organised all the time, because there are times when your psycholigical mood won't allow that ... Or you should be a robot then :) ... All you can do is:
- Try to wake up early & sleep early even you have nothing to do.
- Always be arranged with your meals, don't eat too much, don't let yourself be so hungery.
- Do things as if they will be evaluated internationally.
- Always think logically, think about causes and consequences.
- Be organised in you own room, your own body.
- Accept criticism with an open mind.
- Respect your own opinion as others' opinions and don't feel shy to say it.
Well, I'm sure there are many more things to do but these are what crossed my mind now, forgive my bad English as its not my first language

start with one by one. get habitual to one good habit at a time. start with simple ones like keeping your shoes on place everytime u come home, keeping ur books on place.
u can also allot time slots for performing particular task. eg. some people take too much to take a bath say half an hour or so. this is waste of time. u can take a stop watch and count time u take for performing particular task. then u can decide on how much time u should actully spend on that. and then try doing that task in that particular time period.
a person can become discipline only by practice. that is only by practising good habits. pick up one practice, be habitual and go to next good habit.

practice. When things are unorginized, fix it.
for discipline, practice. The easiest way is just to do or not do something when you dont want to. For instance: You have a bad itch on your leg. dont let yourself scratch it. Works on discipline. Little stuff like that. other stuff like doing your hw when you dont want to helps without external encouragement. the best way is to do something that requires tons of it (like a sport with a discipline oriented coach, or military training, or similar).

Make lists for everything...I am the queen of being anal and organized. I make lists for everything...packing, planning my week or just planning every step to get something done. The key to being organized it to plan it out.
