OK, I have serious internet addiction and my school grades/personal life has taken quite a hit because of it last year.
I really want to cure myself especially since my Junior year in High School is coming up, and I do not want to mess up this important time in my life. Even if I have tons of things to do, I like to stay on the internet for hours - watching anime/speedruns of games on youtube, reading forums, fanfiction, random stuff on wikipedia, just really time wasting things. I can even spend a whole night and a whole day just in front of my computer. Please help me. Now I've got tons of school hw, and studying to do before school starts in September, and I am really desperate!

Basically I would set aside a different place to study and make that your study area. Also, do pleasurable things there like sit at the desk and play games or whatever but not on the computer.
With this separate area, when you're studying take breaks in between, but do something other than go on the computer, like eat a snack or something.
Just have a separate desk, associate it with something pleasurable, don't go on the computer untill you are satisfied with the amount of studying you have done.

Have your parents put a lock on the 'puter. Tell them not to tell you da password no matter how much you ask. And if you need it for something make sure most of your work is done and it's the last thing you have to worry about. Just train yourself to stop. Do all your homework and as a reward go on for a few minutes. Or find a hobby. Don't screw up your life because of a stupid box with a keyboard. It ain't worth it.

You could:
1) Get another healthy addiction to replace it. For example,comics,tv,girlfriend/boyfriend,sports,travel.
2) When you're doing revisions or homework,go out to places without any chance of getting near to a pc within the vicinity.
3) Terminate your internet access, but that is a little extreme.
4) Draft a time table to restrict your lifestyle.
5) Get a lower speed running modem/pc,so your access to fun website would be limited.

Draw up a daily timetable...apportion an appropriate amount of time to your school work and social activities. Never surf the Internet until the important issues are settled first. That way, you get to continue with your Internet activities and stay guilt free.

Go and do work in your local library. I find that with a change of scenery I don't even think about checking my emails. I'd also strongly consider cancelling your internet at home - move back to dial-up 56k, you'll soon get fed up of it! Also tell your parents - they'll help by being strict with you and realising how bad your problem is.

Find another avenue for entertainment, assuming you go to the internet to entertain yourself. Find a new hobby. For me, I usually either play my favorite sport, read my favorite book and eat my favorite food! Hope for the best! And stick to your priorities!

Develop new habit, it is hard to do, you need discipline. Divert your mind attention to one thing youcan do, sports you like perhaps. 21 successful attempts creates a new habit, anew likes and a new views.

Simple get your *** out of the chair and turn the computer off. Of course if you don't do it voluntarily, they'll eventually turn it off that's what happens when you get kicked out of school or in the case of an adult when you don't pay the bill. What you call addiction, I call lack of impulse control. There are people that can walk away from bad habits, they just realize that they control their life not an inanimate object.
