Are you asking what the causes are of kidney disease? If so it varies. My kidney disease was brought on by Henoch Scholein Purpra- an autoimmune disease. There are other causes, sometimes the cause is unknown.

If you are asking what causes someone to need a transplant then that occurs when their kidney function is at less than 25%. Blood work can be done to test for the kreatin level which gets higher as the kidneys get worse, mine is at 456 and I am not on dialysis yet- however my doctor is trying to get a transplant in place before I need dialysis.

Hope that helps.
