Dogs need distemper pareunfluenza hepatitis and parvo (kind of like the 6 in one) every 2-3 weeks until they are 16 weeks. They only need lepto in the vaccine once at around 12 weeks. If a dog is going to react to a vaccine it will be the lepto. At which time they get a rabies vaccine. They need a stool check on the final visit to be sure they are parasite free. They need to be put on heartworm preventative at 10 weeks. A good flea control like frontline, or advantage should be put on at 8-10 weeks.

It will need a total of 4 of the combo shots, given 3-4 weeks apart. Our pups are wormed when they get their shots. In Tennessee the rabies shot is given at 3 months. Other states may differ.If you start it on heartworm preventative, it takes care of all the worms.

Alot of vets vary slightly on it but I work at an animal hospital in maryland so this is how it is for us:
8 weeks-first Distemper (these are combo vaccines that include distemper, hepatitis, adenovirus, parainfluenza, parvo, -optional lepto and corona)
12 Weeks-Second Distemper, First Lyme if desired, Bordetella if necessary
16 Weeks-Third Distemper, Second Lyme, 1 year Rabies(after this first Rabies expires in a year, rabies is given every three years)
Heartworm Preventative should start immediately and should continue monthly for the rest of your dogs life.
For intestinal parasites-there are different types of dewormers for different parasites and directions vary for each one.
Need to check a stool sample and will need to check one yearly also at annual well visits
ps distemper may or may not be given annually after the first one-that is for you and your vet to discuss and decide what is best for you and your pet's situation. Also if you do choose to vaccinate for lyme and bordetella they are annual vaccines. Heartworm tests vary depending on the hospital you choose-ours tests every other year if they are on preventative year round like they should be-if not we test every year

If you take your dog to a veterinarian they will set you up with a schedule of required shots. They usually send you reminders in the mail.

heartworm pills once a month, flea medication every month, microchipping(personal preference) and check with your city, you may have to license him, something that could save you a lot of money in the long run. as far as the shots, they're usually done once a year.

Your best answer would be from your vet, vets know it all. And he could check if you need other medications such as heartworm tablets.
Ask your vet and he'll tell you what you need.

in areas where parvo is common, you should get a straight parvo, and if your puppy is outside or runs much, have the vet check for tapeworms. answer 1 is good
