I want to get her something that is really meaningful.

Gift certificate
There are candels that have from age 1 to 21 on them. you burn them each year on their birthday (halmark)
music box
blanket with babys name on it
baby boot which can be engraved with babys name
baby book
bible
anything that can be saved and the child will have when he/she gets older

Get her something for herself, something she can pamper herself with. When I was pregnant someone gave me something like this, lotions, scented soaps, etc, in a pretty container, and she also gave me some pretty stationary. It's easy to feel overwhelmed when you are about to have a baby, and it's nice to know that people still recognize you as having needs and wants too. I loved everything I got for my baby, it was the most beautiful time in my life, and the nice gift from my friend that was special just for me, topped everything off nicely.

P.S.- Another good idea is to give her a dress if the baby is a girl, a shirt if the baby is a boy. Get it in a grown up size, and say it's for the baby. Then every year, she can put the baby, then child, then teenager in this certain outfit and take a picture, this way she can create a picture diary of how her child grows.

Is this a first baby or does she have other children? If other children, give her a certificate that states you will babysit for them for x amount of days so she can have alone time with the new one. If this is a first time baby, give her a certificate that you will bring in one evening meal a week for a month, or you will be over to help do laundry and clean her house the first two weeks she is home from the hospital with the new one. Another suggestion, you could set up a savings account for the new baby and put whatever denomination you feel you want in, so they could add to it during the next 21 years.

Buy your BFF something that is thoughtful but very cheap. Not cheap as in the dollar store, something that you made, like a pretty basket with baby things in it! Plus it wouldnt cost much and you're being creative and showing off your skills! Many ppl would buy a basket with tons of baby things, but you should take the smart way out and save money!! She would totally go head over hills when she see how creative you are.

All these other ladies have really great ideas, but no one can ever have enough diapers... trust me. Even if you have to buy a larger size the baby will grow into them. Also a gift cert. so Mommy can shop for her new little one at a really nice store is always appreciated. (6 weeks at least at home with a newborn and "everyone else on earth has a life but you" can get really boring)

Get her something that can be engraved when the baby is born. It could be a music box or a picture frame. I just bought my neice an engraved snow globe. If you already know the sex of the baby and the name you could get it partially engraved now and do the date later.

Give her some specially made coupons for you to babysit for a "New Mom's day out." A gift certificate for "Marry Maid Service," A gift certificate for a gym, A standing blanket holder.

Ideas:

picture frame with clay to preserve baby's handprint/footprint (available at the Discovery Channel Store or online)

CDs of soothing music for babies or Baby Einstein DVDs

Something she probably doesn't have and isn't even thinking about needing, but will are picture frames and gift certifactes to get baby's puicture taken. I NEVER thought about that while I was pregnant, but after I had my daughter it was VERY inportant to document her stages with pictures and that cost adds up quickly!!

a baby can never have too many stuffed animals; a shelf for baby's room with something stencilled on it; a painted stool or bookcase; an afghan - one of those throws that you can get special stuff printed on it - like the family's name, maybe.
