Yes, you may pick it up discreetly.

An observant waiter may do it for you.

You can ask for a new one if you like

Also, if you leave the table for any reason, you lay your napkin on your chair, unless you have completed your meal, then you fold it and place it beside your plate.

This tells the waiter whether you have finished eating.

In theory, you shouldn't even have to ask. If you are in a fancy restaurant, your server should place your napkin on your lap for you, and keep an eye on you during dinner. If anything falls, a fork, a napkin or whatever, you leave it there and s/he replaces it for you.
If your plight escapes their attention, you can ask for a new one.

Ask for a new napkin, but do not pick it up. Let the waiter or bus person know it fell and let them get it. Otherwise you will need to wash your hands (who knows what has been on the floor).

You can pick the napkin up.It's only when you drop an eating utensil(fork,knife,spoon) that you should ask for another.

Pick it up and ask for another one. Sometimes the wait staff will rush to pick it up for you, but don't let it lay there for all the other customers to see.

Etiquette says that you ignore it and let the waiter bring you another without making any request. If it's a fancy place, they are trained to notice things like that.

At a REALLY good restaurant, the server would have known it happened before you did and replaced it with a new one!
