Elements of Style is great, no question, but maybe a little limited in scope for what you were asking.

For me the absolute best ever is Warriner's English Grammar and Composition. They're out of print, but you can find them on eBay and half.com. They had different ones for each grade level, so you'd want to look for the Complete Course. The older editions were better than the last ones they published. The one I have is 1977, and I'm happy with that edition.
