People tend to ignore anonymous anything. If I have a complaint about something a hospital did or didn't do I would not only verbally complain, I would also complain in writing in a letter form with a return signature required

It all depends on how serious your complaint is. You can call them and speak with administrators, head nurse, or chief of staff, or chief of department. If the matter is serious enough, you could call the state regulators or the health department, You could also call the insurance company you deal with.
I will tell you, however, if you are not willing to be identified, your complaint will likely not be treated seriously, as they have no way to follow up on your complaint or verify the facts. They also can not get back to you, so there is no real incentive to take any actions.

The better business bureau will be able to direct you as where to go. Most times you need not file a complaint anonymously though, you just need to let them know that you don't want your name revealed. Document everything so it stays fresh in your mind though. Good luck.

they should have a comment box near the main office (usually by itself rather than inside the hospital, but not all hospitals are made the same...) and just fill a complaint but dont put in your name. and if they wont accept the complaint, put in something like "Jane Smith" or another made up name.
good luck.
